Progress Reports
Foster parents should submit a progress report for each child under their care, depending on that child's requirements and level of care. Usually, progress reports are required weekly, every other week, or monthly, but this can vary. Check with your organization to determine how often you should complete a progress report.
Creating a Progress Report in the ThriveWell mobile app
- In the ThriveWell mobile app, select the Progress Report option from the main menu.
- Choose which child the log is for:
- Indicate the range of dates that the progress report is for, then tap Next.
- Indicate if the the child receives Treatment (TRT) services. If so, select the appropriate options.
- If the child is on a safety plan, select Yes, then indicate which one and provide any relevant details or updates.
- If the child requires supervision, select Yes, then indicate the kind of supervision and any relevant details.
- If the child receives nursing care, select Yes, then indicate the name of the agency providing the care and how many hours per day or week are provided.
- If the child uses any special equipment, supplies, or attendant care, select Yes, then provide any relevant details.
- Indicate how the child's appetite has been.
- Indicate if the child has special feeding needs, and provide any supporting details.
- Indicate if there were any voiding problems, and provide supporting details.
- Indicate how the child's hygiene has been.
- Indicate whether the child needs assistance with daily grooming, and what the need assistance with. If they don't need any assistance, select "None, independent" in the second section.
- If the child is school-aged, indicate if they have an educational portfolio, and provide any relevant details and progress during the report period.
- Indicate whether the child receives special education services, and if so, include any relevant details.
- If the child is over 16, indicate any independent living skills they are working on.
- If the child is employed, include any relevant details about their employment.
- If respite was used, provide any relevant details.
- Review the log to ensure all answers are correct, then tap Submit. The log will immediately be sent to the child's case manager.
