Minor incident reports
The Minor incident report can be used to track and report any incident that is not considered "major". Please note: if you need to report a major incident, use the appropriate form provided by your agency.
Included information:
- Date and time of minor incident
- Description of incident (including all relevant details)
- Cause or trigger of the incident
- Consequences
- How the foster parent responded to the foster child's behaviors
- Notifications
Final Step: Review and Submit the log
The minor incident report will be sent to your case manager as soon as it is submitted.
