Behavior logs provide a daily reflection on how the day went for the parent and child. Responses are collected using a five-point scale (Hard, So-so, Average, Good, Great), accompanied by open comment boxes for more context.
Included questions:
- Day rating
- Parent mood
- Noted behavioral issues
- Biological family contact (Yes/No)
- Child mood
- Medication changes (Yes/No)
Final Step: Review and submit the log
Once submitted, the behavioral log will be sent directly to your case manager.
